The Washington & Jefferson College Sports Medicine Department strives to provide professional care and wellness to the health of all Washington & Jefferson College student-athletes.  Our goal is to return our student-athletes to full participation as quickly and safely as possible.

All medical insurance/history forms are located on the links on the left side of this page.  For the 2007-08 academic year, all necessary forms must be downloaded online, filled out and sent back to Mike Lesako prior to August 5 of each year.  Student-athletes will not be able to participate without the completion of these required forms. 

FRESHMEN/TRANSFERS/FIRST-YEAR PARTICIPANTS

All freshmen/transfers/first-year participants are required to complete and return by mail (see below address) the past medical history questionnaire and proof of insurance forms.  The athletic training staff also requires that parents/guardians and student-athletes review the "Letter to Parents/Guardians Regarding the College's Insurance Policy" located also on the links to the left of this page. 

RETURNING STUDENT-ATHLETES

All RETURNING student-athletes are required to complete and mail the proof of insurance form ONLY.  The athletic training staff also requires that parents/guardians and student-athletes review the "Letter to Parents/Guardians Regarding the College's Insurance Policy" located also on the links to the left of this page.
