For your convenience, we've made all of the forms you need to complete as a new W&J student available for you on this website. By completing and returning the following forms, you will be helping us to prepare for your participation in our LAUNCH and Orientation programs, as well as prepare for your arrival to campus in the fall. To help you keep track of the forms as you complete them, we have provided you with a Form Completion Checklist, which is available on this page.
Also, as you review the forms, you will see that some of the forms are available to complete on the website, while others will require you to print them out before completing and returning them.
