Directions: Please read the following carefully. This is government paperwork and if done incorrectly, they will not return your original payment.
NOTE:
If you are an AmeriCorps Community Fellow or America Reads volunteer, do not apply for these clearances on your own!! They will be paid for from the provided grant or budget respectively and will be completed en masse during your first training.
1) PA Request For Criminal Record Check: Online Application

Reason for Request: Volunteer OR Education

Use a legitimate email.

Use your home address (NOT 60 S. Lincoln! NOT a P.O. Box!)

Use your credit card to pay the $10 fee. You are not “Fee Exempt”, you are “Individual”.

PRINT A COPY OF YOUR RECEIPT as proof of purchase.

Results will be sent to your home address from the PA State Police with a PA State Seal in the background entitled Response for Criminal Record Check. Copy or scan this document and bring the copy to the Office of Volunteer Programs for your records (bring the original if you would like us to keep it for you). Without it, you cannot work with school-aged children.
2) PA Child Abuse History Clearance: Paper Application

Use INK only! And if it is not legible, you cannot submit it.

Use your full legal name and your home address. (NOT 60 S. Lincoln! NOT a P.O. Box!)

Purpose of Clearance: Check ONLY ONE Box—“Employment with a significant likelihood of regular contact with children”

If you have more than 6 household members, attach a typed page to the back.

Print this (2 pages) and bring it to the Office of Volunteer Programs for your records. Without it, you cannot work with school-aged children.

Child Abuse History Clearance Waiver: In order to expedite the process, sign the attached Office of Volunteer Programs waiver stating that the processed report will not go to your home address, but will come straight to the OVP mailbox. You can request to keep the original once it’s been copied for your records.
3) FBI fingerprinting: 2 part process

Part 1--Come to the Office of Volunteer Programs in the Hub with the following:

Completed Child Abuse Clearance form and Waiver and receipt for Criminal History Check. (Once the processed Criminal History Check is returned to you, a copy must come to the Office of Volunteer Programs.)

Check for a total of $38.75 made out to “Washington & Jefferson College”. In the Memo, please write the program you are requesting the clearances for (i.e. America Reads, Big Brothers Big Sisters, President’s Prep, etc.)

You will sign up online to get FBI fingerprinted. Do not leave without this receipt!

Part 2--Bring the receipt and your license (or passport) to Mail Boxes Plus More (123 Washington Rd.—behind the big post office across the street from Wash High, looks like a trolley station) during normal business hours (9 a.m. – 5 p.m.) to get your fingerprints taken.

Results will be mailed to your home address from the Pennsylvania Department of Education—School Services Unit. The second page will tell you whether or not you’ve passed inspection. Copy or scan this document and bring it to the Office of Volunteer Programs for your records (bring the original if you would like us to keep it for you). Without it, you cannot work with school-aged children.
